Potholes and broken up tarmac surface including 2 potholes I tripped on near pinchpoint

Several circular holes near Ryde library, broken up surface in tarmac outside 97 George Street down to 91 The Foyer. All breaks are over 16 mm. Broken up surface has been washed away in storms possibly blocking drain through pinch point on library side. How do I claim for injuries and pain? Cannot see road surface as street lamps too dark now and in own light when crossing at pinch point. I believe friends have reported fall. One hole marked for months but nothing done about it. Haven't filled the ones reported.
