Road around Manhole Cover breaking up

Despite being initially reported 14 days ago on this site, this problem on a main road has not been resolved and is now breaking up further with debris being scattered on the road surface and the main hole cover now physically moving. The manhole cover is directly in the line of road traffic wheels and vehicles are now swerving around the holes in the road surface and sizeable debris. I am trying to phone in this issue as well.
